Organization was previously known as CPHC-Community Family Health Team. The LGCFHT offers a wide array of primary health care services supporting health promotion, disease prevention and chronic disease management through its two clinics in Brockville & Gananoque. The interdisciplinary model of Community Family Health Team ensures increased access to various health care supports through the integration of physicians, nurse practitioners, registered nurses, physician assistant, social workers, dietitians, diabetes educators and psychiatry. this approach ensures that high quality, patient-focused care is provided to our patients.
